Partnership in Action for Trinity Health (PATH) is a local group of committed community members and organizations working together in Trinity County, California to improve the health of all Trinity County residents. The PATH coalition was created in 2017 to foster collaboration between members of the community and organizations that want to build a healthier and safer Trinity County.  PATH is facilitated by Human Response Network in collaboration with our valuable members and partner agencies.
